E-PORTAL ACCESS AGREEMENT

Please read this e-portal access agreement carefully before accessing the E-Portal. By continuing, you agree to all terms and conditions below.

IMPORTANT NOTICE

PLEASE CAREFULLY READ THIS ENTIRE AGREEMENT BEFORE YOU USE ANY E-PORTAL SERVICES. THIS AGREEMENT CONTAINS PROVISIONS THAT LIMIT OR EXCLUDE OUR LIABILITY TO YOU. BY ACCESSING OR ATTEMPTING TO ACCESS ANY E-PORTAL SERVICES, AND/OR BY ACCEPTING THIS AGREEMENT, YOU ACKNOWLEDGE AND AGREE THAT YOU HAVE READ AND UNDERSTAND THE TERMS OF THIS AGREEMENT AND HAVE AGREED TO BE BOUND BY ITS TERMS. IF YOU DISAGREE WITH THE TERMS OF THIS AGREEMENT, YOU MAY NOT USE OR ATTEMPT TO USE ANY E-PORTAL SERVICES.

DEFINITIONS

In this Agreement, the following terms and expressions will have the following meanings:

  • Access Device: Any device allowing you to use or access the E-Portal Services, including, but not limited to, personal computers and hand-held web-enabled devices.
  • Account: Any account you have with Central Valley Medical to which you request access through the E-Portal Services.
  • Common View: The ability to view or transact on accounts of multiple business entities or individuals using the same User ID and Password.
  • Electronic Instruction: Any instruction regarding an Account or E-Portal Services transmitted through an Access Device.
  • E-Portal Services: Any products or services available through portions of our website requiring a User ID and Password.
  • Password: Your personal E-Portal password, as amended by you from time to time.
  • Web Site: All webpages and files posted by Central Valley Medical under www.morethancpr.com.

USE OF SERVICES

You may only use the E-Portal Services to access authorized Accounts and conduct transactions you are permitted to perform. You agree not to use the E-Portal Services for any illegal or improper purpose or to undermine the security, integrity, or operation of the system. You must comply with all applicable laws and regulations in connection with your use.

WARRANTY OF AUTHORITY

If you are accessing the E-Portal Services on behalf of a business entity, you warrant you have full authority to do so and to legally bind that entity to this Agreement.

USER IDS AND PASSWORDS

You agree to keep your User ID and Password absolutely confidential. You are solely responsible for all activities conducted under your credentials. If you suspect misuse, notify Central Valley Medical immediately at
1-866-391-6283 (Monday–Friday, 8:00 AM–5:00 PM PST) and change your password promptly.

SECURITY

Central Valley Medical employs reasonable measures to protect confidential information but assumes no liability for viruses, malware, or unauthorized access. You are responsible for maintaining secure devices, up-to-date antivirus software, and safe computing practices.

PRIVACY

By accessing and using the E-Portal Services, you acknowledge and accept Central Valley Medical’s Privacy Policy, as amended from time to time.

LIMITATION OF LIABILITY

Central Valley Medical’s total liability for loss or damages under this Agreement is limited to direct damages not exceeding $50.00 USD. Under no circumstances shall Central Valley Medical be liable for any indirect, consequential, or punitive damages.

GOVERNING LAW

This Agreement shall be governed by and construed under the laws of the State of California, USA. You irrevocably submit to the jurisdiction of California courts for any disputes arising under this Agreement.

CHANGES TO AGREEMENT

Central Valley Medical may update this Agreement at any time by posting changes on this website. Continued use of the E-Portal constitutes acceptance of the updated terms.

TERMINATION

Central Valley Medical may suspend or terminate your access to E-Portal Services at any time and for any reason.

SEVERABILITY

If any provision of this Agreement is held invalid, the remaining provisions will remain in full force and effect.

BY CLICKING “I AGREE,” YOU AGREE TO BE LEGALLY BOUND BY THE TERMS AND CONDITIONS OF THIS AGREEMENT.
